RN/Registered Nurse Hospice

Experienced RNs needed for per diem shifts / after hours

Job Title: Registered Nurse
Reports to: Hospice Director
Supervises: CNA’s / HHA’s

Responsibilities:

  • Provide care as defined within the scope of the State's nurse practice act.
  • If directed by the Hospice Director, makes the initial evaluation visit and regularly reevaluates the patient's nursing needs.
  • Initiates the plan of care and makes necessary revisions, keeping the Hospice Director updated on the revisions. Participates in IDG meetings with hospice team.
  • Provides those services requiring substantial and specialized nursing skill.
  • Initiates appropriate pain and symptom management nursing procedures.
  • Prepares clinical, progress and summary of care notes.
  • Informs the Hospice Director,  physician and other personnel of changes in the patient's condition and needs.
  • Counsels the patient and family in meeting nursing and related needs.
  • Administers agency policies and procedures.
  • Assists the Hospice Director with assessing the competency of clinical staff to promote quality, continuity and safety of a patient's are.
  • Supervises, educates and coaches other nursing personnel as directed by the Hospice Director.
  • Assures proper maintenance of clinical records in compliance with local, state and federal laws.
  • Identifies and participates in agency performance improvement activities.
  • Stays informed about changes in the field of nursing and home health care and shares such information with appropriate organization personnel.
  • Promotes hospice referrals in the health care community.
  • Participates in on-call rotation

Qualifications:

  • Registered nurse with current unencumbered licensure to practice professional nursing in the state of Michigan.
  • 1 year experience in hospice
  • Possess exceptional nursing skills.
  • Comply with accepted professional standards of practice.
  • Excellent interpersonal skills.
  • Excellent customer service skills and the ability to deal tactfully with patients, staff and community.
  • Treat patients, staff and the general public with courtesy, respect and presents a positive public image.
  • Ability to travel on a daily basis.
  • Understands regulatory compliance and Federal regulations including but not limited to OSHA & HIPAA
