Custom Care from the Start

AdvisaCare will work with you, your family, and any other Medical Advisors to design your own Plan of Care. Maintaining and improving your quality of life is top priority.

Step 1: Assessment

Our RN Care Manager will meet with you and conduct an evaluation of your current status. This will include health issues, injuries, other available support, Activities of Daily Living and any other issues critical to your continued safety and well-being.

Step 2: Care Plan

Based upon a careful analysis of your situation, we will develop a custom Plan of Care. Your Plan of Care will be coordinated with any other health care advisors you may be working with. Additionally, we will communicate with any family members or friends you would like to have involved with your care.

Step 3: Caregiver Orientation(s)

Your Caregivers (Nurses, CENAs, HHAs, etc.) are carefully selected based upon skills, personality, availability and your continued input. The Caregivers are provided with the orientation to meet and exceed your expectations. Additionally, your Caregivers' skills are updated with any changes related to your continued safety, security and independence.

Step 4: Ongoing Monitoring

Our Patients receive regular visits that include:

  • Weekly Assessments
  • Monthly Staff meetings
  • Update/Changes to Plan of Care as needed
  • Monthly Updates, and as requested, to all other professionals as directed
