Home health care provides physician-directed medical and rehabilitative services in a patient’s home. Unlike non-medical personal care, home health services are provided by licensed or qualified healthcare professionals and are intended to address specific medical, nursing, or rehabilitation needs.
A home health plan of care may include skilled nursing, physical therapy, occupational therapy, speech therapy, medical social services, and home health aide support depending on the patient’s needs and eligibility.
Our skilled nursing team provides clinical care, education, monitoring, and support in the home. Services may include medication management and education, wound care, disease management, post-surgical care, assessment and monitoring, and coordination with physicians and other members of the care team.

Speech-language pathology services may help patients experiencing difficulties with speech, communication, cognition, or swallowing. Treatment is tailored to the patient’s individual needs and functional goals.
Medical social workers help patients and families navigate challenges that may arise during recovery or while managing a health condition. This can include connecting families with community resources, assisting with care planning, and providing support related to social and environmental factors affecting care.
When included as part of an eligible home health plan of care, home health aides may assist with personal care needs under the supervision of the clinical care team.
Home health may be appropriate for individuals who need skilled care following a hospitalization, surgery, illness, or injury, as well as those managing chronic or complex medical conditions.
